For example the cancer journal, Annals of Oncology has noted that the active ingredient in cannabis can improve the appetites and sense of taste in cancer patients. AIDS patients who received the synthetic cannabis pill dronabinol gained 10 pounds on average after a year of taking it. This indicates that medical marijuana for appetite stimulation can reverse weight loss after it has begun.

The body’s natural endocannibinoids, similar to the cannabinoids found in medical cannabis, help regulate eating so doses of medical marijuana encourage individuals to take in more calories than they might have otherwise wanted. In fact, weight gain brought about by MMJ may actually be higher than that expected based on the calories consumed by patients, a finding that is helpful for many suffering uncontrolled weight loss due to chronic illness.

Researchers in Canada ran a small pilot study between May 2006 to December 2008 in 21 adult patients with advanced cancer who had been eating less as a result of their illness for two weeks or more. Eleven patients received oral capsules containing delta-9-tetrahydrocannabinol (THC) – the main psychoactive ingredient in cannabis – and eight patients were given placebo capsules. The active capsules contained 2.5mg of THC. According to the published study, the researchers found that the majority (73%) of THC-treated patients reported an increased overall appreciation of food compared with patients receiving the placebo (30%) and more often stated that study medication "made food taste better" (55%) compared with placebo (10%).

The majority of THC-treated patients (64%) had increased appetite, three patients (27%) showed no change, and one patient's data was incomplete. No THC-treated patients showed a decrease in appetite. By contrast, the majority of patients receiving placebo had either decreased appetite (50%) or showed no change (20%). THC-treated patients also reported side effects that included better quality sleep and relaxation than those in the placebo group
